PRIVACY ACT
The Sydney Eye Specialist Centre is committed to providing exceptional customer service and care. We acknowledge our obligations to you under the Privacy Act of 1988 (as amended). Personal information we collect from you will be used primarily to ensure that you receive optimal care, but may be released under appropriate circumstances to the following:
- Your referring doctor, optometrist or other medical practitioner involved in your care. This includes other specialists requiring reports on your care as requested by you or your health care professional.
- Medicare - for billing and medical rebate purposes
- Private Health Funds – for billing and medical rebate purposes
- Department of Veterans Affairs – for billing and medical rebate purposes; Veterans Transport
- Pathology and/or X-ray Services – to provide medical diagnoses and appropriate treatment
- Australian Receivables Limited – debt collector for outstanding accounts
- The Private Hospital Data Bureau and NSW Department of Health - statistical information collected when undergoing surgical procedure and used to track inpatient statistics for the purpose of health planning and to meet NSW Health Department and Private Hospitals Bureau data collection requirements.
- Lions Eye Bank, Designs for Vision, Alcon-companies providing prosthetic devises for eye surgery
- The Sydney Eye Specialist Centre for research purposes. Anonymity is maintained.
TELEPHONE POLICY
The Sydney Eye Specialist Centre will, at times, wish to contact you by telephone regarding your eye treatment or confirm or advise changes to your eye appointments. We will endeavour to speak to you personally on the telephone numbers you provide. However, if we are unable to speak to you directly, we will leave a voicemail message or a message with the person answering the call. If you do not agree with our telephone policy, please advise us as soon as possible.
